RELEASE AND CONSENT
In consideration for my name and/or photograph and/or quote and/or story and/or video being selected for inclusion in informational and marketing material produced on behalf of Woodmont College, with third party support, in print, electronically and on the Woodmont College public website, I hereby:
GRANT Woodmont College the non-exclusive and perpetual and worldwide right, license and privilege under copyright or any other right or license enjoyed by me to use, edit, translate, reproduce, publish and distribute my name and/or photograph and/or quote and/or story in print form and/or video form and to use, broadcast, cablecast and reproduce and distribute the above on the Woodmont College public website and by e-mail or other electronic means. I agree that I will not at any time make any claim for additional compensation in respect of such use and waive any right to inspect or approve the finished photographs, quotes and stories and I waive all rights of integrity and all other moral rights.
I FURTHER GRANT CONSENT to Woodmont College to use and disclose my name, photograph, quote and/or story in print or video form, by e-mail or other electronic means and in an online version on the Woodmont College public website (the “Website”).
I FURTHER warrant that I am the author of the story and that I have the exclusive title to the story and/or photograph and/or video and that the story and/or photograph and/or video do not infringe upon or violate any copyright, moral or other right of integrity, any other intellectual property rights or any personal privacy rights of any person. I agree to release and forever discharge Woodmont College and its officers, agents and representatives from any and all claims, demands, expenses, actions, causes of action and or any and all liability howsoever caused, arising out of, or in any way related to the collection, use and disclosure of my photo and/or quote and/or story and/or video authorized to be collected under this form.
I have read this Release and Consent before signing below, and I understand the contents. I understand that by giving this Consent, I am permitting personal information about me to be used in print form, by e-mail or other electronic means and in an online version on the Website. I also understand that if consent were withheld none of these uses would occur. I also understand that Woodmont College has no control over and is not responsible for the use or misuse of all or any part of this testimonial by third parties who access the Website.
I have submitted my name and/or photograph and/or quote and/or story and/or video to Woodmont College voluntarily and I have given my consent voluntarily.
